Chloroquine is now uncommonly used in favor of its derivative hydroxychloroquine. In the United States, hydroxychloroquine is most often used for its anti-inflammatory effects in rheumatology and dermatology.

Apr 20, 2011 Problems with Plaquenil Use. Toxic maculopathy associated with chloroquine use was first documented in the literature five decades ago.1 In the United States, Plaquenil––an analog to chloroquine––is used to treat a variety of conditions, including rheumatoid arthritis, lupus and several distinct inflammatory disorders. Most cases of eye disease occur in people receiving more than 400 mg of Plaquenil or more than 250 mg of Aralen daily. Atabrine is not known to cause retinal damage. Recent evidence shows that there is no irreversible eye damage with Plaquenil in doses of 5 mg/kg 2.3 mg/lb per day. IOP elevation is asymptomatic, so it can only be detected with diligent and timely follow-up examinations including applanation tonometry. Steroid-induced IOP elevation typically occurs within 2–6 weeks of beginning steroid therapy. Upon stopping corticosteroids, the IOP usually normalizes in a few weeks to months.

While early toxicity may be asymtomatic, patients with more advanced stage of toxicity may complain of color vision changes or paracentral scotomas. Its toxic effects on the retina are seen in the macula.

Aug 01, 2009 Plaquenil toxicity is not reversible in most cases and continues to progress even after you stop taking it for several month and sometimes years because of the half life of Plaquenil One should see a retina special before taking Plaquenil in my opinion Plaquenil can cause serious side effects when used in high doses for chronic diseases such as RA over the long term. The New Zealand Dermatological Society recommends that to decrease the risk of retina damage, no more than 200mg of Plaquenil should be taken a day, and a dose of 400mg per day should be taken no longer than a few weeks. A recent study conducted in Turkey found that topiramate induced changes in the eye. 5 The study included 76 eyes of 38 patients who were taking topiramate for the treatment of migraine. Patients were examined at baseline and after 15, 30 and 90 days of topiramate treatment. After 90 days of treatment.
